As part of the registration, the Academy shall require the parent(s) to provide copies of the student’s birth certificate, health records if applicable, and Baptismal certificate. Report cards and standardized testing results are required for students transferring into 3rd to 8th grade. (Some standardized testing will be provided by the Academy for enrolled students or those who don’t have any records yet.)
The Academy is a voluntary educational program, supplementing homeschool families or functioning as a “sole vender”, providing a full suite of courses to families. As such, immunization records are not required for those who are participating “part-time” (3 days a week or less). Immunization records should be submitted to the front office for those who are participating more than three days a week.
